
A lot of this list is single-run conditions that don’t conflict with each other, so you can earn several at once instead of replaying for each. Each plan below is one run. Achievements in a plan are marked with a ⛓ badge further down.
After your first clear, start a new save and switch on both Hard and Iron modes. Kill every Titan - not just the minimum needed for the gates - then beat the game without dying once.

Nesting by wording: an Iron + Hard clear also beats the game in Iron Mode (Iron Human) and beats Hard Mode (New Game+). This is the hardest way to do it; the safer split is an Iron + Hard file that only fights the Titans you need (Iron Titan, Iron Human, New Game+) and a separate normal-difficulty Iron file for all Titans (Iron God). Conflict: Hard Bizkit cannot share this file - a player on the Steam forums reports No Roll cannot be enabled together with Hard.
Start a new save with No Roll on (and Hard off). Fight only the Titans needed to open both gates, in the order you are fastest at, and go straight for the finale.

Both only ask you to beat the game, so a fast No Roll clear can earn both. Losing the roll makes some fights slower, so if the 20-minute target is not happening, split them: Aerodynamics is allowed in any mode and is easier with rolling.
Kill your first Titan. Every Titan dies to a single arrow in its weak spot, so the whole game is about reading patterns and finding the opening. Unmissable.
Story progress: beat the first set of Titans and open the first Sealed Gate. Unmissable.
Open the second Sealed Gate. A community guide notes you only need some of the Titans (seven of the twelve) to open it, so you can leave the ones you are struggling with for later.

A community guide reads this as killing the Yeti Titan in under 10 seconds. Learn its opening pattern and take the first clean shot at its weak spot. Quick to retry.
Land the killing arrow on the Skull Titan during its spinning attack rather than in a quieter moment. Practise the timing — a death is a fast retry.
In the final battle for the Titan Soul, get hit by your own arrow so you knock yourself out. Guides describe it as hitting yourself during that fight.
The description is just '???'. Community guides describe a secret final boss that you reach after defeating all the other Titans; one guide places it in the Grave Ruins area. Exact trigger and location not verified across two sources — see a community guide before you hunt for it.
Kill the Rolling Titan within five seconds of the fight starting. Stand where you can see the opening immediately and take the first clean shot.
Shoot out every tooth on the Mountain Titan first, then finish it. Don't land the kill early by accident.
In the Knight fight, destroy every pillar in the arena before landing the killing shot.
During the fight in the iceberg arena, destroy every iceberg before finishing the Titan. A community guide places this in the ice boss fight.
Shoot off every vine on the Plant Titan, then kill it. Same idea as Dental Plan: clear the extras first, kill last.
The Chest Titan spits out coins as you hit it. Keep the fight going until 50 coins have been produced, then finish it.
In the Lava Blob fight, get a bomb to land on its head. Exact source of the bomb not verified — see a community guide.
The Sludge Titan splits when hit. Keep splitting it until the pieces can't divide any further, then make the final kill on the last piece.
Same trick as Laser Eye Surgery, on the Broken Statue Titan: the kill has to happen while you are drawing the arrow, not from a released shot.
Spend a total of 30 seconds under the effect of spores (from the mushroom Titan's area). It is cumulative, so just keep getting hit by spores across attempts.
Light your arrow on fire before the killing shot on the Brain. Find a fire source in the arena, pass the arrow through it, then take the shot.
Beat the game with Hard mode switched on. Hard, Iron and No Roll modes unlock after your first clear and are switched on when you start a new save file. Hard can be combined with Iron (that is what Iron Titan asks for), so you can earn both in one file if you are confident.
