Flowers are off-path collectibles. Look in side rooms, behind hay bales, near corpses. First 6 are scattered across early-mid chapters. Wiki has the full list.
Specific chapter has a hidden tomb off the main path. Look for graveyard sections; the tomb is in a sealed crypt. Wiki has the location.
Early in the game, Rodric (your blacksmith friend) offers sling practice. Complete the optional training mini-game. Easy to skip if you rush past.
An NPC is mortally wounded; use Ignifer or other alchemy to save them. Requires recognising the rescue option in the moment. Wiki has the exact scene.
Mid-game scene: a soldier on the ground about to be killed by rats. Use Extinguis (extinguish fire) on the nearest torch to attract rats away, saving him. Achievement on his survival.
During a quiet chapter, Hugo asks for food. Find a bread/fruit pickup in the area and bring it to him. Story-incidental.
Sling has multiple upgrade tiers via crafting materials. Find enough scrap throughout chapters. Fully maxing it = achievement.
Curiosities are story-flavour items (skulls, trinkets, parchments) hidden off-path. 13 cumulative.
Multiple equipment tracks (pouch slots, alchemy capacity, item carry limits). Max each via crafting workbenches found in chapters. Need to find all upgrade workbenches first.
Mid-game has a stealth-or-frontal-assault option at a fortified location. Choose frontal (the hard way). Combat-heavy alternative path; achievement on completion.

Alchemy carts are special loot crates with valuable materials. 5 in total. Some are off-path; look for the carts marked with alchemy symbols.
Late chapters introduce orphaned children Amicia's group cares for. Find hidden gift items (toys, small treasures) for each. ~5-7 gifts total.
Don't get separated from the Captain during his escort segment. Stay within a few meters. Distance breaks the achievement.
