A lot of this list is single-run conditions that don’t conflict with each other, so you can earn several at once instead of replaying for each. Each plan below is one run. Achievements in a plan are marked with a ⛓ badge further down.

1One Shivering Isles run, start to finish· 8 achievements

Install the expansion, take the strange door in Niben Bay and play the Court of Madness questline through to the end without stopping. Every rank below is awarded for reaching a step on that one ladder, so they arrive in sequence in a single run - there is nothing to repeat and no second character needed. Go in geared: the Isles are balanced well above the level at which the door first appears.

2The one choice you must save before· 2 achievements

Make a manual save immediately BEFORE the conversation in which Sheogorath appoints you Duke. Take one side, let the achievement fire, then reload that save and take the other.

CONFLICT, and the only real trap in the game: Mania and Dementia are mutually exclusive outcomes of a single dialogue choice, so they cannot both land in one uninterrupted run. They do not need a second playthrough either - one manual save before the appointment covers both. They are the two rarest achievements in the game precisely because most players pick a side, keep going, and never realise the other was recoverable.

3One Arena ladder· 9 achievements

Sign up at the Arena in the Imperial City and fight the matches back to back rather than drifting off between them. Repair your armour and restock healing between bouts, since the higher ranks assume you arrive in fighting condition.

Thieves Guild promotion works differently from the other factions: as well as doing the guild's quests, you have to move a running total of stolen goods through the guild's fences, and the next job only opens once you have hit that quota. So keep stealing and selling between quests - burgling houses at night and fencing the loot is the fastest way to clear the requirement. Note that killing innocents can get you expelled, and you have to pay your way back in. This one is the end of the questline rather than another quota step.
