The tutorial achievement. Build a GP's Office, a reception desk and one treatment room, hire the staff to run them, then let the first patient walk the loop from diagnosis to cure. It pops the moment a single patient is cured, so it cannot be missed in Hogsport.
Hospital level rises as you earn reputation and progress, and Level 10 arrives naturally partway through the early campaign hospitals. Keep curing patients, keep your reputation climbing and take on the level's objectives rather than idling. There is nothing special to do beyond playing a hospital through its first couple of stars.
Cumulative across your whole career, not a single hospital, so this ticks over on its own as you play the campaign. If you want it faster, run a hospital with plenty of GP's Offices and cheap diagnosis rooms so throughput stays high and queues do not stall.
Machine rooms (Pharmacy, Ward machines, the cure rooms) can be upgraded by a Handyman with the Mechanics qualification. Select a machine, choose the upgrade option and let a handyman perform it. Each individual upgrade counts once, so 25 upgrades spread across a few hospitals gets there quickly. Hire at least one or two dedicated Mechanics handymen so upgrade jobs are not competing with repairs and cleaning.

When a patient dies in your hospital their ghost can haunt it, and a Janitor with the Ghost Capture qualification can remove it with a vacuum. Train up janitors in Ghost Capture and make sure at least one is always free. This is cumulative across your career, so it accrues over a long campaign - deaths happen, and every ghost captured counts.
Every hospital in the campaign has three star objectives; each star earned counts toward this total. Fifteen stars is roughly the first stretch of the campaign map. Note that stars are cumulative across all hospitals, so finishing early hospitals to 3 stars before moving on is the most efficient route.
Twice the Level 10 requirement. Hospital level climbs with reputation and progress, so this needs a hospital you settle into for a long run rather than one you leave at 1 star. Later campaign hospitals with large plots and long objective lists are the natural place for it.
Kudosh is the meta-currency earned by completing the in-game challenges and objectives listed in the Kudosh panel. It is account-wide and never resets, so it accumulates over your whole time with the game. Keep an eye on the Kudosh objective list and steer your play toward whatever is currently listed rather than grinding blind.

Two Point Hospital's multiplayer challenges are asynchronous competitive scenarios you opt into from the menu rather than live co-op. Enter one and play it through to its conclusion. Because these are time-limited events, availability depends on what is running when you play - check the challenge board in the main menu.
Superbug Projects are the community/co-operative research goals accessible from the Superbug Lab. Contribute to and finish the project named "Hair Removal" specifically. Like the other Superbug content this is tied to the lab rather than a single hospital, so progress carries across your saves.
Organisation Value is a running score across your whole organisation, driven by hospital value, reputation, cash and the size of your estate. Fifty million is a late-career figure: it needs several hospitals built out well past their star objectives, kept profitable and kept expanding. Focus on high-value rooms, upgraded machines and full plots rather than rushing new maps.
Build Research Departments early in each hospital and keep them staffed with Doctors who have the Research qualification. Research runs continuously, so the real bottleneck is having enough research desks and qualified staff. Fifty completed projects is comfortably reachable across a handful of campaign hospitals.
Double the County-Wide requirement. Keep three-starring hospitals as you unlock them rather than moving on at one star - it is far faster to finish a hospital you already understand than to set up a new one.
Staff qualifications go up to five levels in a speciality; the fifth is the world-class tier. Build a Training Room, assign a Doctor to the Research course repeatedly and keep training them until the Research qualification is fully maxed. Training is faster with a high-level trainer, so recruiting or training a good instructor first pays off.
Same process as the researcher: put a Doctor through the Psychiatry course in the Training Room until the qualification hits its top level. Psychiatry is worth levelling anyway because psychiatrists both diagnose and cure, so this doubles as a genuinely useful hospital investment.
Awards are handed out at the end of each in-game year and each covers a different category. Winning all of them in one hospital means that single hospital has to be strong across every measured area rather than specialised. Because only one award is given per year per category, this takes many in-game years in a hospital you keep pushing rather than leaving. Aim to keep hygiene, staff happiness, cure rate and finances all high simultaneously.
Build a Marketing Department, staff it with an Assistant and keep launching campaigns back-to-back. Each completed campaign counts, and campaigns are cheap relative to their benefit, so leaving marketing running permanently in every hospital gets this without any detour.
The third rung of the star ladder. By this point you are three-starring most of the base campaign map. Nothing new to do - just keep completing every star objective on every hospital you unlock.
Max out the Surgery qualification on a Doctor via the Training Room. Surgery is a multi-level speciality, so expect several training courses back to back. Keep the trainee off other duties while they train so the courses run without interruption.

A regional achievement: every hospital in the Pointy Mountains region of the campaign map needs at least its first star. One star is the cheapest objective on each map, so this is really about unlocking and touching every hospital in the region rather than perfecting any of them.

This is tied to the time-portal content, so it needs a hospital where portal patients arrive, and 100 of them cured inside a single in-game year. The practical approach is a hospital built for throughput before you start counting: plenty of GP's Offices, short walking distances, duplicated cure rooms for the common portal illnesses and enough doctors that nothing queues. The year resets the count, so set the hospital up first and only then let the year tick over.
A later star milestone. Same route as the earlier star achievements - three-star everything as you go. If you are short, revisiting an earlier easy hospital and finishing its remaining objectives is usually quicker than starting a hard new one.
Frogborne is one of the game's named illnesses, so you need its specific cure room built and staffed in a hospital where that illness appears, then see one such patient all the way through diagnosis to cure. Illness distribution varies by hospital, so if it is not showing up, move to a map where it does and keep the relevant treatment room open.
The Herb Garden is a treatment room, so build it, staff it, and let a patient whose diagnosis routes to it be cured there. As with any single-room achievement, the room has to actually be the one that performs the cure - having it built but unused will not count.

A regional achievement for the Pebberley Island hospitals. Get every hospital in that region to at least one star. As with the other region achievements, one star per map is the goal, not three.
Eco Rating is driven by how your hospital generates and consumes power and how much waste it produces. Maxing it means leaning on the greenest available power generation, upgrading it, and keeping the dirty options out of the hospital entirely. Because the rating is a running measure, you need the green setup in place and left running rather than switched on briefly.
R.E.M.I. X is the remixed, harder rerun of campaign hospitals. All three named hospitals - Hogsport, Lower Bullocks and Flottering - have to be completed in their remix versions. They use the same maps you already know, so the difficulty is in the tightened objectives rather than unfamiliar layouts; bring the layouts that worked first time round.
Another star milestone. Nine more stars than the previous tier. Keep completing objectives across every unlocked hospital, including the DLC regions if you own them.
Alien Infiltrators are tied to the Close Encounters content, so you need a hospital in that region running and the means to identify and expose them. Fifty is a large number, so this is a long-run achievement in a hospital you keep going rather than something you set up and finish in one sitting.

Regional achievement for the Off The Grid DLC, which is built around power generation and self-sufficiency. One star at every hospital in that region. Requires the DLC.
Regional achievement for the Culture Shock DLC. One star at every hospital in that region. Requires the DLC.
Four specific head-themed illnesses, all cured in the Clockwise-above-Thyme hospital from A Stitch In Time. That means having every one of the four relevant cure rooms built and staffed at once, then waiting for a patient of each type to arrive and see their cure through. Keep all four rooms permanently open rather than swapping them in as needed, since you cannot control which illnesses walk in.
Regional achievement for the A Stitch In Time DLC. One star at every hospital in that region. Requires the DLC.
Another nine stars on from the previous tier. At this level you will almost certainly need the DLC regions to have enough hospitals available.
The second R.E.M.I. X set: Mitton University, Tumble and Flemington all completed in their remixed versions. Same principle as World 1 - familiar maps, tighter requirements.
The Yesterizer is a machine specific to Clockwise-above-Thyme in A Stitch In Time, and the achievement counts cured patients sent through it. A hundred is a throughput problem: build the hospital for volume, keep the machine upgraded and repaired so it is never out of service, and keep the run going rather than restarting the map.

Nine more stars again. By this point the practical advice is simply to finish every remaining objective on every hospital you own, base game and DLC.
Mudbury Festival is a wave-based scenario, and Wave 18 is deep into it. The run has to survive continuously, so failure means restarting the wave sequence rather than reloading a step. Build for resilience: redundant cure rooms so one breakdown does not stall everything, plenty of handymen for repairs, and staff rooms so nobody quits mid-run.
Research nodes are the individual items on the research tree. Fifty completed nodes needs sustained research capacity: multiple research desks, doctors qualified in Research, and hospitals kept running long enough for the tree to advance. It stacks with Well-Informed, so build research heavily once and both progress together.
VIBE is the Culture Shock rating measured at Fitzpocket Academy. Push whatever the hospital's VIBE panel is asking for and keep it high rather than spiking it briefly - the rating is evaluated on an ongoing basis. Requires the Culture Shock DLC.

Chasm 24 is an Off The Grid hospital and the Robo-Janitor prototypes are a set unlocked over the course of that map. Every prototype in the set has to be activated in the same hospital, so plan on a long run there rather than leaving at one star. Requires the Off The Grid DLC.
Ninety stars. Essentially a completionist marker for the campaign plus DLC - keep clearing objectives everywhere.
Overgrowth is a hospital where land plots are bought and unlocked over time. Unlocking all of them means running the hospital long enough and profitably enough to afford every plot, so treat it as a money problem first: get the hospital solidly in the black, then expand steadily rather than overbuilding early.
Windsock is an Off The Grid hospital built around power. Maximising green energy capacity means building out the greenest generation the map offers to its limit and keeping it running. Requires the Off The Grid DLC.
Plywood Studios is a Culture Shock hospital and Roderick Cushion is a named character there who levels up. Getting him to Level 5 means committing to whatever the hospital's progression system asks over a long run in that map specifically. Requires the Culture Shock DLC.
The league tables rank your hospitals against the rest of the world across several categories, and this needs first place in all of them simultaneously and held for three consecutive in-game months. That means one exceptionally strong organisation rather than several average ones - high value, high reputation, high cure rate and strong finances all at the same time. Realistically an end-of-career achievement.
Another named Superbug Project from the Superbug Lab. Contribute to and complete the project titled "Learning Machine Learning" specifically.
Ninety-nine stars. At this point you are three-starring nearly everything the game and its DLC offer.
Topless Mountain is a wave-based scenario and Wave 42 is deep into it - a very long uninterrupted run. Build for endurance rather than speed: redundant rooms so nothing is a single point of failure, plenty of handymen, staff rooms and training kept up so staff do not burn out, and finances stable enough that you are never forced to cut corners mid-run.
The highest star tier in the list. This effectively requires three stars on essentially every hospital across the base game and the DLC regions.
The Pantomobile is one of the Speedy Recovery ambulances. Twenty patients on a single trip means the vehicle has to be upgraded for capacity and dispatched when there are enough waiting patients to fill it in one run. Upgrade the ambulance first, then time the dispatch rather than sending it out constantly. Requires the Speedy Recovery DLC.
Every ambulance type from Speedy Recovery has to exist, and be fully upgraded, in one hospital at the same time. That is a sustained money and time commitment in a single map - buy each vehicle type, then take every one of them to maximum upgrades before moving on. Requires the Speedy Recovery DLC.
