Stagger an enemy (build up the white stagger gauge with charged/heavy hits or perfect guards) then press the heavy-attack prompt for a Fatal Attack. You'll do this constantly — your first comes in the opening hours.
Stargazers are the bonfire-equivalent checkpoints. Repair the first one early in the game — unmissable tutorial step.
Hidden story-boss achievement. The Parade Master is the first major boss. Perfect-guard its big swings, dodge the leap, and punish after combos. Unmissable on the critical path.
At certain story dialogue prompts you can tell the truth or lie. Choose the lie option the first time it's offered (e.g. the early hotel/clinic conversations). Lying steers you toward the human/Real Boy ending.
Perfect-guard (block at the last instant) repeatedly against a single enemy to chip their weapon's durability until it breaks. Find a slow heavy enemy and perfect-guard its attacks until the weapon shatters.
Stalkers are aggressive elite enemies that ambush you in certain areas. Defeat one — they hit hard, so perfect-guard and punish during openings.
Hidden story-boss achievement. The Scrapped Watchman has an electric second phase — bait its slow swings, perfect-guard the combos, and stay mobile when it electrifies. A common difficulty spike for new soulslike players.
Hidden story-boss achievement (Fuoco, the fire boss). Bring fire-resistance, attack its legs, and watch for the ground-slam fire bursts. The Flamberge/charged attacks stagger it well.

Dimensional Butterflies are special enemies that drop quartz/upgrade materials and flee. Kill each colour/type at least once — track them across regions; they appear in fixed spots.

Reach the 5th (final) phase of the P-Organ by collecting enough Quartz across the whole game. Pick up every Quartz you find on each run to fully unlock it.
Hidden late-game boss achievement (Laxasia). A brutal two-phase fight — Phase 2 opens with a hard-to-time lightning combo. Learn the perfect-guard rhythm and bring lightning resistance. One of the toughest in the game.
Hidden endgame boss achievement (Simon Manus). Multi-phase with an Ergo-storm second phase — keep moving through the AoE, perfect-guard the long combos, and use a high-DPS weapon to race the phases.
Hidden 'last story' achievement for a character revealed late in the game (likely tied to the DLC/Overture or an endgame reveal). Complete their storyline. We'll confirm the exact character and update this.
Take any normal weapon to its maximum level using upgrade materials at Venigni's bench. Funnel your materials into one weapon.
Special weapons use a different (Ergo) upgrade path. Strengthen one boss weapon to its cap — save up the special upgrade materials for it.

After obtaining the relevant record/sheet, play the piano in Hotel Krat and hit the rhythm prompts perfectly. Practise the timing — a clean run unlocks it (and a reward).
Complete the blacksmith Eugénie's questline at Hotel Krat (bring her the items/people she needs) to reach her final story.

Cryptic Vessels are puzzle containers tied to the cipher/Trinity content. Solve each and claim its reward — most sit in or near Trinity Sanctums.
Hidden final-boss achievement (the Nameless Puppet). The hardest fight in the game — extremely fast, perfect-guard-dependent combos. Learn the timings in pieces, keep your weapon's durability up, and stay aggressive between strings. Beating it finishes the game.
