"Light bonfire flame." This is the kindling action at a bonfire, not just resting at one: kindling raises the number of Estus charges you get there and costs a humanity, so you need at least one humanity in your counter while you are in human form. Kindling also requires the Rite of Kindling for anything beyond the first level, but the first kindle at any bonfire is enough for this achievement.
"Acquire Estus Flask." You receive the Estus Flask during the opening Undead Asylum section, so this unlocks as part of normal story progress.
"Arrive in Lordran." Story progress: finish the tutorial asylum and you are carried to Lordran, which unlocks this.
"Ring Bell of Awakening at Undead Church." One of the two Bells of Awakening; reaching and ringing it is required story progress, so it comes naturally. The route there runs through the Undead Burg and the Undead Parish and includes a boss fight on the church roof.
"Discover Way of White covenant." Covenant achievements pop when you actually join the covenant (accept the oath when its NPC or object offers it), not merely when you find it. You can leave straight afterwards - the achievement stays - but leaving can anger some covenant leaders, so if you plan to come back read up on that covenant's rules first. The Way of White is one of the first covenants offered to you, from an NPC who sits near the Firelink Shrine early on.
"Ring Bell of Awakening in Quelaag's domain." The second Bell of Awakening sits beyond the boss of Quelaag's Domain, reached through Blighttown. Required story progress, so it comes with a normal playthrough.
"Arrive in Anor Londo." Story progress after both bells are rung and Sen's Fortress is cleared. Unlocks on arrival.
"Acquire the Lordvessel." You receive the Lordvessel in Anor Londo after defeating the Ornstein and Smough boss fight. Required story progress. Note that placing the Lordvessel changes some NPCs, including the Darkwraith covenant leader, so if you want Covenant: Darkwraith on this cycle read that entry before placing it.
"Acquire the Rite of Kindling." This item lets you kindle bonfires to higher levels and is a boss reward in the Catacombs. Bosses only give their rewards once per cycle, but the boss is mandatory for the Lord Soul route through the Catacombs and Tomb of the Giants, so a full playthrough picks it up.
"Discover Chaos Servant covenant." Covenant achievements pop when you actually join the covenant (accept the oath when its NPC or object offers it), not merely when you find it. You can leave straight afterwards - the achievement stays - but leaving can anger some covenant leaders, so if you plan to come back read up on that covenant's rules first. This covenant is joined in Quelaag's Domain after the boss there; exact requirements (and what can lock it out, such as the fate of the NPCs involved) are best checked in a community guide before you go.
"Acquire the Art of Abysswalking." This item lets you survive in the Abyss, which you need to reach the Four Kings, so it is required for the full route to the ending. Where exactly it comes from is not described by Steam - trigger not verified here, see a community guide.
"Defeat Seath the Scaleless, inheritors of souls." One of the Lord Soul bosses that must be beaten to finish the game, so a full playthrough unlocks it. Seath's Lord Soul fight takes place in the Crystal Cave; read up on the fight's gimmick before going in.
"Defeat the Soul Lord Gravelord Nito." A mandatory Lord Soul boss at the bottom of the Tomb of the Giants, so this unlocks on a normal playthrough. Bring a light source for the Tomb itself.
"Defeat the Four Kings, inheritors of souls." A mandatory Lord Soul boss in the Abyss, reached through New Londo Ruins once you hold the Art of Abysswalking. High damage output matters because the kings keep arriving on a timer.
"Defeat the Soul Lord Bed of Chaos." The final mandatory Lord Soul boss, found at the bottom of Lost Izalith. Story progress, so it comes with a full playthrough.
"Discover Princess's Guard covenant." Covenant achievements pop when you actually join the covenant (accept the oath when its NPC or object offers it), not merely when you find it. You can leave straight afterwards - the achievement stays - but leaving can anger some covenant leaders, so if you plan to come back read up on that covenant's rules first. "Defeat Crossbreed Priscilla, the Lifehunter" Priscilla is an optional boss inside the Painted World of Ariamis, which you only reach by carrying the Peculiar Doll. Because the area is optional it is easy to skip on a cycle - if you finish a playthrough without visiting it, achievement progress is kept across New Game+, so anything you miss on this cycle can still be picked up on the next one. Priscilla can also be talked to rather than fought; for the achievement you need to defeat her.
"Defeat Dark Sun Gwyndolin, the Darkmoon God." Gwyndolin is an optional boss in Anor Londo, so it is easy to finish a cycle without fighting him. Fighting him also affects the Darkmoon covenant, so if you want Covenant: Blade of the Darkmoon on the same cycle, join it first. "Reach 'To Link the Fire' ending." One of the two endings. Endings are chosen at the very end of the game, so a 100% needs at least two final fights - one for each ending - and most players take one ending, carry on into NG+, and take the other next time.
